Any waste electrical and electronic equipment (WEEE) item which is to be disposed of or serviced on site must be cleaned and decontaminated in the appropriate manner. Please fill in the form below to create a record of decontamination.

Completed forms are automatically sent to the Estates Helpdesk.

The acknowledgement email from the Estates Team should be printed out and taped to the waste ready for when site services come to collect the equipment.
