Whether you wish to attend/ reject/ or request a different interview date you must complete the reply slip below.
Physiotherapy reply slip (Word)
(This must be emailed to admissions@sgul.ac.uk as an attachment).
Candidates must print off and complete the following documents. These documents will be collected from you on the day of the interview.
- Student Entry Information (Word)
- SGUL CRB Disclosure Form (Word)
If you need these documents in a different format or require a hard copy these can be requested via admissions@sgul.ac.uk.
Please be aware that failure to supply documents on time may result in the withdrawal of your place irrespective of good examination results.
